a. 50 year old
restrained driver sustained a “ whiplash “ type of injury. She reports
a brief loss of memory “ retrograde “ type of amnesia at the time of
impact.

Brain MRI and CT angiogram are
unremarkable. MRI of the cervical spine revealed C 6-7 mild
central stenosis and posterior annular tear of disc. EEG was normal.
c.
Neuropsychological testing revealed mild
impairment of attention, severely impaired auditory memory for
immediate and mild for delayed recall, moderate impairment with visual
delay recall, moderate depression.
d.
Clinical problems continue and include a
decrease in word finding skills, sense of dizziness, brief episodes of
decreased attention “ zoning out “ and a painful feeling of “ pressure
in her head “ that is near constant. Headache treatments have been
disabling with excess sedation or disequilibrium from medication.
Acupuncture continues to be helpful.

SPECT imaging revealed a significant
decrease in activity along the left Prefrontal cortex and Parietal
lobe bilaterally, focal increase in the Thalamus and lateral Temporal
lobe.

Treatment for her head pain as “
headache “ has not been successful. Ongoing acupuncture and analgesic
medication is currently prescribed. She has not agreed to take
medication for depression.
